Please read the following terms and conditions before submitting a Moment of Magic nomination.

  • If you provide a valid email address, you will receive an automated email with a copy of the text of your nomination.
  • Your nomination will be checked by a member of the Communications Team at Hull University Teaching Hospitals NHS Trust. If it is suitable for publication, it will be approved within 48 hours (Monday-Friday) and displayed on official channels owned and administered by Hull University Teaching Hospitals NHS Trust. These may include: Pattie (the hospital staff intranet); our public-facing website; our social media channels; hospital news bulletins and newsletters; the monthly chief executive briefing; and trust board papers. This information may include:
    • The name(s) of the individual(s) or team(s) the submission is about
    • The name of the department or ward where they work
    • The details of what they did
    • When the submission was made
    • Whether the submission came from a staff member, a patient or a relative

The name and contact details of the person making the submission will not be published.

  • A monthly report of this information (not including the submitter’s contact details) will also be shared with the Risk Management Team who administer the Greatix scheme. The aim of this scheme is to identify excellent practice within the hospital and share it with other services.
  • All nominees will receive a “Someone thinks you’re amazing” card in the internal post within 8 weeks of the nomination.
  • Your name and contact details will be stored securely and will not be displayed to anyone except the Communications Team. The exception to this is if any patient safety or safeguarding concerns are raised as a result of your submission, in which case your contact details may be passed onto the Risk Management, Safeguarding or Patient Experience teams, who may contact you.
  • If the nominee makes contact with the Communications Team to ask who nominated them, the Communications Team will contact you to ask for permission to share your name and contact details with them. We will never share your name and contact details without your permission.
  • Every month, all nominations are read by a group of between 10-20 members of staff from across the trust and a shortlist of 3 is made. From this shortlist, the Chief Executive chooses one Moments of Magic winner for that month. The individual or team that wins for that month will be presented with a certificate at an annual presentation ceremony. If your submission wins, and you’ve provided your contact details, you will also be invited to the ceremony.
  • The winners of the monthly Moments of Magic will go forward to be considered for a Golden Hearts Award at our annual staff awards ceremony.
  • You can, at any time, request a copy of any Moments of Magic which contain your personal information. This includes where you are the nominator, or where you are the nominee. You can also request that this information is deleted. Please report any concerns to
